How Do I File a Medical Claim?

In many cases, your physician or other health care provider will file the claim for you. If not you’ll need to submit a letter to file a medical claim to your health insurance company.

 

What Do I Need to File a Claim?

 

To File a Medical Claim, you’ll need the following:

  • Your insurance policy or benefits booklet - the policy literature should include instructions on filing medical claims

  • A claim form from your insurance company or managed care organization

  • A statement from your insurance provider to submit along with the claim form

  • A cover letter to accompany the claim form and statement

Your insurance provider will also need more detailed information about the medical services and care your received, so be prepared to supply the following information:

  • Your policyholder medical ID number

  • The nature of the injury or illness

  • The diagnosis and procedure codes

  • The type of service(s) you received

  • The date(s) of the service

  • The charge(s) of the service(s)

  • Your provider’s identification number
